Should Putnam Studio Have to Cancel Your Workshop *

I understand that should Putnam Studio have to cancel the workshop, I will be offered: 1) a full refund (less associated Stripe, PayPal, and processing fees if any), and release of your space in the workshop. (Must be requested within 30 days of cancellation announcement. Make your request by email to support@loriputnam.com); or, 2) if rescheduling of your workshop to a future date occurs, you may choose to have all previous monies collected applied to the new dates and your space reserved.

Waiver of Responsibility *

Upon submission of this form I hereby release Putnam Studio, Lori Putnam, and Mark Putnam from any responsibility for prepaid travel costs incurred should my workshop be cancelled due to unforeseen circumstances including but not limited to: natural or manmade disaster, medical emergency, epidemic, or unexpected crisis.

Waiver and Assumption of Risk *

Upon submission of this form I hereby unconditionally release, waive and discharge my right, whether by contract or under operation of law to file cause of action(s) or claim(s) which I may have against Lori Putnam, Putnam Studio, or any of its associates now or in the future. I hereby assume any and all risk of loss, liability, damage or costs, including bodily injury or property damage that may incur arising out or in connection to my acts and/or omissions. I fully understand the terms set forth in this form, and I hereby waive my rights freely and voluntarily without any inducement, assurance, or guarantee being made to me at the fullest extent allowed by law.

I/we are willing to wear masks unless otherwise instructed that it is safe to remove them.

I/we will follow health and safety protocols such as hand washing/sanitizing and social distancing.

I/we understand that social events and dinner may or may not go on as previously scheduled. (This will be determined closer to the time of your workshop.)

I/we feel safe and comfortable attending this workshop.

Additional Non COVID-19 Cancellation Agreement *

I understand that if I register for a 2021 workshop, agree to the above statements, and decide to cancel at a later date, I will expect to be reimbursed in accordance with the Standard Cancellation Policy (pre-COVID) for Tennessee workshops: 1) if cancellation is made: more than 60 days prior to workshop start, I will receive a full refund less $100.; 2) 30 to 59 days prior to workshop start, I will be refunded one half of your workshop fee.; 3) less than 30 days prior to workshop start, I will receive no refund. NOTE: Associated Stripe and PayPal processing fees, if any, may also be deducted from your refund.
